Understanding member enrollment and attribution as it relates to patients you have seen or not seen at your health center is one of the foundational supports for success in your ACO. Integration of plan and EHR data in tools that easily accessible allows you to identify patients that have never been seen at your organization and those who have not recently engaged allowing you to target populations of interest based on the organization’s goals. We will review how you can evaluate patient/member matches to improve your match rate, compare EHR vs Plan PCP assignments and plan to primary payer. Lastly, will explore how to assess clinical and health related social needs data for members that are patients at your organization.
