Turning SDOH Data into Impact: Empowering Health Centers to Screen, Track, and Act

Social Drivers of Health (SDOH) screening has become a standard workflow for many Massachusetts health centers, but screening alone isn't enough. True impact requires the ability to track needs over time and take meaningful action at both the individual and population levels.

In this webinar, we’ll explore how to use Azara DRVS to move beyond screening and into impact. Learn how to identify SDOH barriers, improve screening rates, and connect patients with essential resources and social services. We’ll walk through how SDOH data is mapped within DRVS and share best practices for ensuring your data is clean, accurate, and actionable. Participants will also be introduced to a new SDOH measure developed by the Mass League to help health centers monitor screening rates and stay aligned with state requirements. In addition, we’ll review tools that support tracking and managing SDOH needs for both individual patients and broader populations.
