CHW Forum: Housing 101 for CHWs in MA

The League's CHW Intiatives team is excited to offer our first CHW forum of 2025! 

This session will cover Housing 101 for CHWs in MA and will be facilitated by Jamie Blackburn, Senior Program Manager at CSH (Corporation for Supportive Housing). 

Learning Objectives:

  1. Understand the position of housing as the leading social driver of health. 
  2. Build awareness of federal, state, and local housing programs, eligibility, and how to access them. This includes: 
    1. Emergency shelter, 
    2. Residential Assistance for Families in Transition (RAFT), 
    3. Federal and state subsidy vouchers, 
    4. Public housing and
    5. Continuum of Care Resources. 
  3. Discuss levels of health and housing partnerships health centers can engage in to advance this work. 
  4. Understand the critical role of CHWs in addressing social drivers of health via community partnerships.
